My private counseling practice is dedicated to maintaining the privacy or your personal health information and I also am required by law to do this. This notice is a shorter version of the full legally required NPP available for your review in the policy and procedure manual located in the waiting area of this office. It describes how medical information may be used and disclosed and also who may have access to this information Please review this summary carefully and if you have any questions, please discuss them with me.
Information may be disclosed in the following circumstances:
- When there is a serious threat to your health or safety or the health or safety of another individual or to the public. (This includes the abuse of a child, elderly person, or a mentally disabled individual) I will only share information with a person or organization that is able to help prevent or reduce that threat.
- Some lawsuits and/or legal or court proceedings
- If a law enforcement official requires doing so
- For Workers compensation and similar benefits programs
There are some other situations like this but they don’t happen very often and they are described in the long version of the NPP.
Primarily I will use the information given by you and from others to provide treatment and arrange payment of services or for some other business activities, which are called in law, health care operations.
